What this sheet is for

Growth monitoring is one of the most consistent tasks in the first twelve months. Health visitors and paediatricians check weight, length and head circumference at every routine appointment, and many parents weigh their baby in between. This sheet puts all three measurements in one place, week by week, so you can spot a plateau or a sudden jump and arrive at any appointment with an accurate, dated record rather than relying on memory.

How to use it

Fill in the Month / Age field at the top right before you begin each sheet. Each row represents one measurement session — write the date on the left, then enter Weight, Length and Head Circumference in the three columns. You can record measurements in metric, imperial or both; just be consistent within each column. Use the Observations & Concerns notes area beneath the table to write anything relevant that week: a new food introduced, illness, poor feeding, or a comment from a health professional. At the foot, fill in Total Gain This Month to see the running picture, and note the date of the Next Clinic / Weigh-In so the sheet doubles as a reminder.

What is printed on the page
