What this sheet is for
Keeping track of which vaccines your baby has had, and which are still due, is easy to lose on top of broken sleep and busy clinic visits. This sheet gives you one place to follow your baby's immunisation schedule across the entire first year, broken into the four main appointment windows that most national programmes use.
How to use it
Work through the four numbered phases in order. Phase 1 covers Birth to 2 weeks and includes birth-dose vaccines and admin steps before you leave hospital. Phase 2 covers the 6 to 8 week appointment, the first big round of injections. Phase 3 covers 3 to 5 months, when the primary course is completed. Phase 4 covers 6 to 12 months, when boosters and the MMR are due. Tick each item when it is done and fill in the Date given column with the actual date administered. Bring the sheet to every clinic visit.

Two things about this particular phased timeline are settings rather than text, and “Baby First-Year Vaccines Checklist” has them set like this. Tips
Schedules vary by country — cross-reference this sheet with your national immunisation programme before each appointment.
Write the vaccine lot number in the date column if space allows; this is useful if a recall ever occurs.
If a dose is delayed due to illness, note the rescheduled date in pencil so you can update it easily.
