What this sheet is for
This sheet is for anyone who suspects their self-care has quietly slipped — not through dramatic neglect but through the steady erosion of small habits. You reach for it when you feel flat, overextended, or out of rhythm and want a concrete, low-effort way to see where the gaps actually are, rather than guessing.
How to use it
Write the month and year in the field at the top right. Each evening, work down the Daily Habit column and tick every box for a habit you completed that day. At the end of the month, count each row and enter the total in the Days column on the right — this makes patterns visible at a glance. Use the End-of-month observations box at the foot to note which habits were hardest to sustain, what got in the way, and one change you want to carry into the following month.
What is printed on the page
“Daily Self-Care Check-In” is laid out as 30-day grid: ten labelled habit rows against 31 numbered day columns of tick boxes, an optional totals column, and a notes box at the foot. Under the title it reads “Track 10 foundational self-care habits every day for one month.”, and the line at the foot of the page says “Missing two days in a row is the warning sign — not one. One missed day is normal; two is a pattern to address.”.
The habit labels slot holds ten entries — “7–9 hrs sleep”, “Drank 6–8 glasses water”, “Moved body 20+ min”, “Time outdoors” and “Ate a proper meal x3” and five more — so the sheet is already filled in for self care rather than left as blank rows to label yourself.
The remaining labels on the page are “Month / Year” for the month label, “Daily Habit” for the habits heading, “Days ✓” for the total heading and “End-of-month observations” for the notes heading.
Two things about this particular 30-day grid are settings rather than text, and “Daily Self-Care Check-In” has them set like this. Totals column is on: add a totals column at the right of the grid. Notes box is on: include the notes box at the foot.
Tips
Tick boxes in the evening, not in advance — honest data is more useful than optimistic data.
If a row total falls below 15, treat that habit as your single focus for the next month rather than overhauling everything.
The 'Said no to one thing' row counts small refusals too — declining a late request or skipping a non-essential task.
